Evergreen Mobile Dental is a non-traditional mobile dental office. Due to this nature, we are an Out-of-Network provider and require full payment at the time of service. We encourage you to confirm with your insurance provider whether Out-of-Network coverage is accepted. As a courtesy, we will gladly assist you by preparing your insurance claims for potential reimbursement from your insurance provider. Please contact us for any questions.
